The regulation of cerebral blood circulation in subclavian steal syndrome.

چکیده
Blood flow to the brain was studied in 11 patients suffering from subclavian steal syndrome. The venous isotope-dilution method was used to measure total blood flow to the brain and the dye-dilution method with intravenous administration of 50 mg of Evans blue was used to determine cardiac output. Cerebral blood flow was significantly lower in the 11 patients with subclavian steal syndrome than in the control group, and the 02 consumption of the brain was low, cerebral resistance was elevated, and the cerebral fraction of the cardiac output was decreased in comparison with the controls. The effect of occlusion of the ipsilateral brachial artery was studied in 10 of the patients with subelavian steal syndrome. The cerebral blood flow and the cerebral fraction of the cardiac output increased significantly as a result of the occlusion in these patients. Studies on three patients after surgical correction of the lesion in the subclavian artery showed significant improvement in the cerebral circulation.

Subclavian Steal Syndrome
The Subclavian Steal Syndrome is a condition that results from the stenosis or occlusion of the subclavian artery proximal to the origin of the vertebral artery. The blockage causes the reversal of the normal direction of blood flow in the vertebral artery which is termed the "steal", because it steals blood from the cerebral circulation. Blood is drawn from the contralateral vertebral, basilar...

ness of the importance of obstructive lesions of the proximal subclavian artery in the production of symptoms of cerebral ischemia. Since this is a form of cerebral vascular insufficiency readily improved or cured by surgery, it is imperative that the "steal" syndrome be suspected clinically and that appropriate diagnostic procedures be performed. Symptoms of brainstem ischemia coupled with bra...

patients are asymptomatic.1 The syndrome of subclavian steal caused by an occlusive lesion of the aberrant subclavian artery is a rare clinical finding.1-4 This syndrome results from the abnormal flow of blood due to the occlusion in the subclavian artery proximal to the origin of the vertebral artery. Blood flow through the vertebral artery is consequently reversed and the subclavian one thus ...
متن کاملBilateral Subclavian Steal Syndrome
Bilateral subclavian steal syndrome is a rare condition. It is usually due to reversal of vertebral blood flow in the setting of bilateral proximal subclavian or left subclavian plus innominate artery severe stenosis or occlusion. This finding may cause cerebral ischemia related to upper extremities exercise. We report a case of bilateral subclavian steal secondary to total occlusion of the inn...
متن کاملFatal cerebral hemorrhage early after subclavian artery endovascular therapy.
We report a case of cerebral hyperperfusion syndrome complicated by a fatal intracranial hemorrhage as a consequence of endovascular recanalization of an occluded left subclavian artery. This entity is thought to be secondary to the failure of the normal autoregulatory mechanism controlling cerebral blood flow after recanalization. Patients at risk for this relatively rare complication are not ...
